Dr. Sam Sherif is the chief dentist at the Cosmetic Dental Centre. With academic qualifications from both the United Kingdom, and the United States of America, and complex trans-Altantic professional experience, Dr. Sherif takes pride in offering the finest standards in patient care.
In addition to his work at the Cosmetic Dental Centre, Dr. Sherif maintains two private dental practices: one near Malibu, California, limited to implant and cosmetic dentistry, and one in London's Harley Street, the United Kingdom's premier medical and dental district.
After receiving his dentistry degree the world famous School of Medicine and Dentistry at Guy's, King's College and St Thomas' Hospitals, Dr. Sherif graduated from Harvard School of Dental Medicine, where he completed a five-year full-time advanced graduate prosthodontic residency. A dental implant is a small titanium screw that serves as the replacement for the root portion of a missing natural tooth.When both the tooth, and the root are damaged, the best permanent replacement is a dental implant in conjunction with a ceramic crown. Treatment with dental implants is complicated, and takes meticulous planning for best results. At most dental practices, an implant team (including either a periodontist or an oral and maxillofacial surgeon, as well as a prosthodontist or a general dentist) is required to undertake the complex work of the implant procedure.

At the first appointment, we use an x-ray to determine if the jaw bone is adequate to hold the titanium posts. Models of the mouth are often made using a soft, pliable plastic compound. These models show where the gaps are located and provide measurements for a replacement tooth or a bridge. The modern implants being placed often only require one surgical procedure using a local anesthetic and sedative.
Is Age a Problem?
Occasionally, older patients express concern that their age may prevent them from enjoying the benefits of dental implants. However, health is more of a determining factor than age. If you are healthy enough to have a tooth extracted, you are probably healthy enough to have dental implants. Certain chronic diseases may contraindicate dental implant treatment. Fat in the abdominal region can often be very difficult to lose, even with strict dieting and fitness regimes. This can affect a variety of different patients, including older patients with slight obesity who may also have poorer skin tone through ageing, and those who have lost significant amounts of weight but have been left with unsightly sagging skin and stretch marks.
Abdominal reduction/tummy tuck, also known as abdominoplasty, is designed to remove excess skin and fat from the middle and lower abdomen, and can dramatically improve the size and shape of the abdomen. As such, it is one of the most popular cosmetic surgery procedures.
In addition to removing the surplus skin and fat, other procedures may be added as necessary, including plication (the tightening of the abdominal muscles), and liposuction to remove excess fat elsewhere on the abdomen and waist. Since this is a major surgical procedure, patients who intend to lose weight in the future, or, in the case of women, intend to have children subsequently, should postpone an abdominoplasty.
There are technical variations on the principle of abdominoplasty:
- Standard Abdominoplasty
The surgeon removes the skin and fat from between the pubic bone and umbilicus, and tightens the abdominal muscles. In drawing the skin at the level of the umbilicus down to the pubic level, the umbilicus itself must be brought through the new skin lying over it. This form of abdominoplasty results in a long, curved scar at the pubic level, and also a circular scar around the umbilicus. The procedure is performed under either general anaesthetic, and usually takes about hours, with a one night stay at the Hospital.
- Modified Abdominoplasty – Mini-Tuck
The excess skin above the pubic bone is removed, and the abdominal muscles are be tightened under the umbilicus, with the umbilicus remaining undisturbed. The procedure is performed under either general anaesthetic, and usually takes about 90 minutes, with a one night stay at the Hospital.

The goal of your surgeon and the Hospital staff is to make your entire experience as comfortable, and worry-free as possible.
VASER Lipo® is the latest technology in body sculpturing and is the alternative to traditional liposuction. VASER Lipo® uses ultrasound energy to break up fat leaving veins, nerves, blood vessels, and surrounding tissue relatively intact. This gives smooth, beautiful results, along with faster healing times, minimal pain, swelling or bruising, and shorter convalescence times.
The procedure melts fat with the ultrasound probe. The fat is then removed with suction and massage using cannulas designed to give minimal tissue trauma. VASER Lipo® is performed under local anaesthetic, and is a day case – not requiring an overnight stay at the Hospital.
Results are immediate, recovery times are measured in mere days, and only minor swelling for approximately a week. As the swelling subsides, the results continue to improve for up to six months, as collagen grows and tightens the skin.
The most body popular areas for VASER Lipo® with women are the abdomen, hips, inner thighs, outer thighs, buttocks, and underarm. With men, the most popular areas are the abdomen, love handles, and excess breast tissue (often referred to as ‘man boobs’). Excess fat in certain areas of the body is often extremely difficult to lose, even with determined dieting, and exercise. Men and women with small localised fat deposits (such as outer thigh saddle bags), and those with significant upper or lower body disproportion often have little or no substantive weight problems, but nonetheless find themselves unable to lose this excess fat. Such fat deposits often run in families, and are resistant to diet and exercise. For such patients, surgery is the only option to remedy this condition.
Liposuction, in all its various guises, offers a straightforward, surgical method of removing this excess fat, and is one of the most popular cosmetic surgery procedures. For most men and women, fat removal results in a dramatic improvement in overall body proportion, and enhanced self-confidence.
- Suction Assisted Lipoplasty
This is the term that applied to a group of operations that are intended, in broad terms, to suck out the excess fat content of localised areas on a person’s body. Their purpose is, in almost all situations, to improve the contour or shape of the body. In this sense, the term can be considered to encompass all the following operations:
- Conventional Liposuction
This was the original form of fat removal, and is the technique used by the majority of cosmetic surgeons in the United Kingdom. It usually involves the use of large cannulae, and conventional suction machines. The aim is generally to remove only the deeper layer of fat. As a result, it is often associated with residual fat deposits.
- Liposculpture
This is a more modern refinement of conventional liposuction. It uses far smaller cannulae, and involves the surgeon suctioning fat from much nearer the surface of the skin. As a result, it leaves far less fat behind, and, in experienced hands, produces much smoother, and more aesthetically pleasing results. Furthermore, by producing such thin skin flaps, skin tightening is much more effective and predictable. The suction may be applied either by machine or in a much gentler fashion by a simple hand held syringe (known as superficial syringe liposculpture). By using such small instruments, the surgeon is able to sculpt the body in a gentle, controlled way.
In conventional liposuction, a vacuum machine is used to help the surgeon remove the fat. In liposculpture, a hand held syringe is used. The surgeon uses whichever technique or combination of techniques that he deems most appropriate for each patient.
The procedure may be performed under either local anaesthetic with sedation, or general anaesthetic, and usually takes between 30 minutes to four hours (depending on the volume of fat to be removed). Many patients go home the same day, whereas larger cases require at overnight stay at the Hospital.
Generally, patients are advised that they may not see significant results within the first few weeks. In larger cases, there will be some size difference and most patients should see an improved shape. Patients wear tight support dressings for several weeks. By around eight weeks, most patients achieve about half of their ultimate visible outcome.
Ears that protrude prominently can be the cause of much dissatisfaction, and even great unhappiness for men and women who experience this condition. Prominent ear correction/reshaping, also known as otoplasty, is a surgical solution that can produce significant change in one’s appearance, and is considered one of the most successful cosmetic surgery procedures.
The primary objective is to reduce the protrusion of the ears, and to provide a normal configuration when the ear is viewed from the side. This is achieved by modifying the flexibility of the cartilage of the ear, thereby altering the shape and position to a more desirable appearance. The procedure may vary, but usually an incision of 4 to 5 cm is made in the skin in the groove behind the ears, where any remaining surface scar will not be directly visible. Occasionally, incisions are made on the front surface of the ear, although these are placed within the normal contours.

As one of the most recognisable features of the face, the nose defines our facial appearance. Many men and woman express dissatisfaction with their noses for a number of reasons, including length, width, the presence of humps on the bridge, and breathing problems.
Nose reshaping, also known as rhinoplasty, can address these problems in many patients, and achieve extremely effective results. The best candidates for nose reshaping are patients with thin, non-oliy skin, those who do not regularly use nasal sprays or other nose medications, non-smokers, and those with realistic expectations about the surgery.
The primary aim of nose reshaping is to reduce the size and shape of the bony and cartilaginous framework of the nose, and to re-drape the nasal skin over the altered shape. Occasionally, the patient may only require surgery to correct a deformity of the tip of the nose in what is known as a rhino-tip procedure. It is generally impossible to fully correct a heavy, bulbous nasal tip, over which the skin may be thick and oily.
There are two basic types of rhinoplasty – closed and open (in addition, the internal septum that separates the two sides of the nose may require straightening, a procedure known as septoplasty):
- Closed Rhinoplasty
Tiny incisions are made in the inside of the nose, leaving no external scars. The majority of nose re-shaping surgeries are of this type. Whilst popular with cosmetic surgeons and patients, it has the disadvantage that the surgeon does not have as good a view of the operative field as with Open Rhinoplasty, and which limits the ability to remodel the bone, and cartilage.
- Open Rhinoplasty
An incision is made on the inside of the nostrils, and across the divide of skin between the nostrils – the columella. The skin over the nose is then lifted, which allows the surgeon a clear, uninterrupted direct view of the framework of the nose. As a result, it is easier to make more extensive alterations to the shape of the nose and to provide a more sculpted nasal tip. However, there is the slight disadvantage of the tiny scar across the columella (which is generally barely visible with time), and the fact that any swelling may take longer to subside than with the closed method.
Augmentation Rhinoplasty
Occasionally, patients present with what they consider to be a flat nose, perhaps as a result of injury, previous surgery, or even an inherited or ethnic trait. In order to improve the appearance of a flattened nose, it is necessary to insert some form of additional framework under the skin of the nose to enhance the line of the nasal bridge.
Several options are available for this additional framework, namely using the patient’s own bone (usually taken from the hip, rib, or surface of the skull), the patient’s own cartilage (usually taken from the ears, or from within the nose itself), or using artificial materials, such as silicone implants.
The procedure may be performed under either local anaesthetic with sedation, but most surgeons prefer general anaesthetic. It and usually takes around one hour to 90 minutes, and can be performed as either a day case, or with an overnight stay.
The operation is surprisingly pain-free, and most patients require only simple analgesia, though patients should expect the nose to feel stuffy for several weeks. There may also be visible bruising around the nose and eyes for a few weeks, and areas of the nose, especially the tip, may feel numb for around a month. Following surgery, ice packs may be used to reduce swelling and bruising. For the first few days after surgery, patients are advised to rest quietly, with only gentle activity. Strenuous activity should be avoided for four weeks. More specific advice is included in the relevant patient literature provided by the Hospital.

You may bring in photographs of personalities with an idea that you would like that particular type of nose, but it must be remembered that, even if the surgeon is able to exactly replicate that nose, it may look very different on a different face

As we get older, it is common to notice changes in the appearance of our eyes. The eye lids may start to sag, or become puffy as early as our early thirties, and the skin may begin to show excessive wrinkles with advancing years. These changes occur because the skin of the eyelids becomes weaker, allowing the fat that naturally cushions the eyeball to fall forward, creating ‘bags’. This has the effect of making the eyes look smaller, and the face look older and tired. These same effects can be seen in younger patients, often in their twenties, who develop excessive skin and fat during childhood or adolescence, and in patients with eye changes associated with thyroid disease.
Eye lid reduction and eye bag removal, also known as blepharoplasty, is a common cosmetic surgical procedure to remedy these symptoms. It is often combined with a brow lift, or a facelift to achieve a more youthful appearance. Surgery to the upper eye lids is referred to as upper blepharoplasty, and surgery to the eye bags is referred to as lower blepharoplasty. When combined, the purpose is to tighten hooded eyelids, restore their normal contours, and to eliminate fatty bulges. The procedure does not correct fine crow’s feet wrinkles – can be treated very effectively through non-surgical options (such as Botox®) – nor dark circles under the eyes caused by excessive skin pigmentation.
On the upper lid, an elliptical incision is made across the crease of the eyelid, allowing the removal of excess skin, and, in some cases, a thin strip of the underlying muscle. In addition, a small amount of fat may also be removed.
On the lower lid, there are two popular techniques. Traditionally, an incision is made just below the lower lashes, allowing a small ellipse of skin and muscle to be removed, along with the small pads of fat. Where there is a fatty bulge, with no excessive skin, a transconjunctival or incisionless blepharoplasty may be possible. An incision is made, hidden just inside the lower eyelid, healing naturally without the use of stitches.

In addition to the several conventional facelift procedures, the Scarless Suture Method (SSM) facelift, also known as the Serdev Scarless Suture Method, offers patients contemplating facelift surgery a further level of choice.
The goal of the Scarless Suture Method is to restore the natural proportions, volumes, and angles of the face by raising the face tissues. As a significantly less invasive procedure, where the sutures actually dissolve and are reabsorbed in the skin, the aim is to achieve better aesthetic results, with less scarring.
Rather than the incisions of other facelift procedures, the Scarless Suture Method involves needle skin perforations between predetermined points in the face. These catch movable but stable tissue, and attach it to non-movable tissue using special polycon stitches. These stitches are elastic, absorbable, antimicrobial, and braided, and lift the target area of the face (namely example, the brow, temporal region, or mid/lower face) whilst preserving the area’s natural mimics and movement. The results are intended to rejuvenate the appearance of the face, while keeping its ‘natural look’.
The Scarless Suture Method can be used in the following facelift treatments (all of which are usually performed as day cases – not requiring an overnight stay at the Hospital – under local anaesthetic):
- Brow Lift
A brow lift using the Scarless Suture Method can improve the appearance of eyelids using less invasive means. Results can be immediate, with minimal or no swelling or bruising.
- Temporal Lift, and Mid-Face Lift
This procedure remedies the sagging of the Submuscular Aponeurotic System (SMAS), a condition that frequently results in a sad and tired expression. In older patients, this is frequently seen as jowls in the mandibular area, or a loss of volume in the mid-portion of the cheeks or in a visible naso-labial fold.
In the temporal lift, the effects of the procedure are seen as a tightening and upward and lateral pull of the temporal region.
In the mid-face lift, lifting loose SMAS in this area give a more prominent cheek-bone, with volumes moved to a higher position, and an elongation of the ‘beauty triangle’.
- Lower-Face Life
Like the Temporal, and Mid-Face Lifts, a Lower-Face Lift can remedy SMAS sagging, restoring aesthetic angles and proportions, resulting in a more youthful appearance in the older patients, and an enhancement in the contours and appearance of younger patients. The age of men and women interested in facelifts varies, with some relatively young patients wishing to reverse the negative effects of their lifestyles on their facial lines, wrinkles, and sagging, while many older patients desire a facelift to make them ‘look good for their age’. Whereas some symptoms of aging and lifestyle can be treated very effectively through non-surgical options (such as Botox®, and dermal fillers), in many instances a fuller and more permanent remedy can best be achieved through a facelift.
The term facelift covers a number of surgical procedures that do one or more of the following: remove excess skin; remove excess fat; tighten loose skin; tighten sagging muscles. There are several different types of facelift surgery, and certain procedures are more suited to certain patients. Also, different surgeons may specialise in one or another according to their surgical preferences. In addition to the Scarless Suture Method, the different types of facelift include:
- Mini-Lift
This is the most basic of facelifts, and involves little more than a skin tuck. The surgeon lifts the skin from the underlying muscle, bone, and fat, then the skin is stretched taut, and any surplus skin excised, and the wound sutured. Drains may be used.
The mini-lift has little, if any effect on the grooves running between nose and mouth (known as the naso-labial grooves). Depending on the amount of skin removed, the results may be more or less marked, and, consequently, last for varying periods of time.
- SMAS Lift
The Superficial Musculo-Aponeurotic System, or SMAS, procedure involves the surgeon lifting the deeper layer of muscle in addition to the overlying skin. This allows the structures in the lower face and neck to be sutured to bone higher on the face, hence tightening the sling running around the lower face and chin/neck. Any excess fat may also be removed.
- Extended SMAS Lift
In this extension to the conventional SMAS procedure, the surgeon lifts the whole of the skin and SMAS layer all the way to the nose, totally elevating it from the cheek structures beneath. Consequently, the appearance of the mid-face is improved considerably, as are the naso-labial grooves.
- Composite Facelift
This procedure is generally taken to involve an extended SMAS lift, to which a browlift is added, and possibly also lower eyelid surgery.
- Mask Lift
This modern technique is a much more extensive procedure, and is sometimes referred to as Subperiosteal Facelift. It involves a hairband-type incision, which is taken right down through the covering of the bone. This allows all of the deeper layers of the face to be lifted. Incisions within the mouth to release all of the structures of the cheek.
- Endoscopic Lift
This is a refinement of the Mask Lift, allowing for the lifting of the deeper structures of the face to be performed through several small incisions, and through key-hole surgical instruments.
A basic Mini-Lift may take around two hours. Larger face-lifts, especially if combined with other procedures, may take around five or more hours. It is likely the face will be bandaged following surgery. Depending on surgeon preference, and the type of lift, these bandages may remain in place for up to five days. For the first 24 hours or so after surgery, patients are advised to sleep upright. Sutures are usually removed after one to two weeks. Rest at home for the first week following surgery is essential, and strenuous activity should be avoided for about one month, though it is important to take gentle exercise.

Modern state of the art of body contouring may involve management of any combination of the following five features:

Male Breast Reduction
For many men, surgery offers the best route to removing excess breast tissue (often referred to as ‘man boobs’), and achieving a regular, flattened male chest. The proportion of men suffering from this condition, known as gynecomastia, ranges from 40-60%. Excess breast tissue in men has a variety of causes , including hereditary factors, changes in hormones, the use of certain drugs, and disease. In adult males, the causes of breast growth can be more complicated, and less likely to resolve without treatment, with one breast being larger than the other, and prone to becoming more prominent due to changes in weight. As the excess tissue is glandular, rather than mere fat, dieting is not a solution.
In contrast to breast removal in women, male breast reduction is a significantly less invasive procedure, known as subcutaneous mastectomy. The surgeon removes the excess tissue through a small skin incision (which can result in less skin scarring), and liposuction. The surgeon will always seek to place incision in the natural contours of the chest, to reduce the appearance of scarring, although some scarring may still be visible. Depending on the patient, excess skin may also be removed to ensure the most aesthetically pleasing result.
Male breast reduction is a relatively straightforward procedure, and the results of the surgery are evident very soon afterwards, with an improved, more slender chest.

Patients should shave the area to be treated either the day before or on the day of the treatment. Due to the many scientific advances in medicine over the last century, people are living longer, healthier lives, but at the same time we may feel that our outside appearance does not always reflect how we feel on the inside. Wrinkles form as our skin gets older and less elastic, and the Sun does its fair share of damage as well. Injectable treatments are a quick, and convenient way to smooth-out lines on the forehead, and around the eyes and mouth, without any need for surgery.

Post-surgery aftercare is critical for the ongoing success of your procedure.
Continuing care is central to our ethos, and a crucial part of the service that we provide to our patients. Patients who have undergone cosmetic surgery need to know that their medical needs will be met, and that their post-operative requirements will be attended to with as much care and precision as their surgery. At the Alexandra Private Hospital, our dedicated team of surgeons and nurses will be by your side through every step of your treatment, from initial consultation until well after your surgery has been completed.
To achieve the best possible results from your cosmetic surgery, all our surgical patients receive free aftercare treatment on-site at our hospital, including follow-up consultations and examinations with your surgeon. Unlike patients of various cosmetic surgery clinics, which may send their patients to different locations after surgery, your aftercare with us will be provided at the same hospital in Chesterfield where your consultation, and surgery were performed.
Following your surgery, you will receive full information on the aftercare treatment that your surgeon and our nursing team will provide. This will include a review session approximately 7 days after surgery. At this time, your surgical wounds will be examined to check that they are healing properly. Your sutures may also be removed or trimmed, and dressings re-applied. Your surgeon will conduct further review sessions with you approximately 6 and 12 weeks after surgery, and arrange any further subsequent appointments as required.
Your health is our priority, and your surgeon will not discharge you until he is satisfied that your aftercare has been completed fully. At any time, you will able to ask your surgeon or the nursing team any questions that you have about the aftercare process, or how you are healing. As with any kind of surgery, it is important to follow your surgeon’s advice regarding your recuperation period. The Hospital will provide you with prescription painkillers, and antibiotics in most instances, and you will be advised to avoid certain activities for several weeks, including smoking, drinking, and strenuous physical exertion. This is all to guarantee your health, and help achieve the best results of your surgery.
In most cases, the results of your surgery will take time to show. Your surgeon will provide you with details on how much pain, swelling, bruising, and possible scarring you can expect. He will also give you advice and instructions on the best way to aid the recuperation process. As your body heals, and swelling and bruising decrease, your new physical appearance will become visible.
VASER Lipo® is the latest technology in body sculpturing and is the alternative to traditional liposuction. VASER Lipo® uses ultrasound energy to break up fat leaving veins, nerves, blood vessels, and surrounding tissue relatively intact. Matters concerning the vaginal region can often be difficult for women to discuss. Many women complain of discomfort when wearing tight clothes/underwear, or during sport activities due to the size and the prominence of the inner labia, which often protrude outside the outer labia. In addition, some women feel self-conscious about the appearance and size of their labia, particularly when there exists a significant difference between the size of the labia. Also, due to a variety of factors, including childbirth/labour, and the effects of ageing, the vagina may lose its tightness, with undesired results that can affect a woman’s self-esteem, and cause embarrassment.
Vaginal rejuvenation in the form of labial reduction, and vaginal tightening provides surgical means of addressing these concerns.
Labial Reduction (Reduction of Labia Minora/Inner Labia)
This surgical procedure, also known as labiaplasty, is performed to reduce the size and the prominence of the inner labia, and to correct asymmetries where these are present. It is accomplished by removing a small portion of labial tissue, generally as a central wedge or a peripheral slice of tissue, which will leave a permanent scar.
Labial reduction is not reversible, and it is generally impossible to increase the size of the inner labia in case of dissatisfaction with the result of the surgical procedure.
Vaginal Tightening
This surgical procedure, also known as vaginoplasty, is performed to reduce the size of the distal (most external) part of the vagina. Successful surgery can both increase vaginal stimulation during sexual intercourse, and boost confidence.
Vaginal tightening is accomplished by removing a small portion of vaginal mucosa from the lateral walls of the vagina in the shape of an ellipse. The stretched or damaged supporting tissues of the vagina, and the muscles of the perineum are restored, and stretched and excess vaginal skin is removed. The effect is an immediate reduction in vaginal size. The removal of tissue leaves a permanent scar that will run mostly inside the vagina, but that will also be visible for a length of few centimetres on the outside of the vagina. The procedure is not reversible.

Fat in the abdominal region can often be very difficult to lose, even with strict dieting and fitness regimes. This can affect a variety of different patients, including women with loose skin and poor muscle tone following several pregnancies, older patients with slight obesity who may also have poorer skin tone through ageing, and those who have lost significant amounts of weight but have been left with unsightly sagging skin and stretch marks.
Abdominal reduction/tummy tuck, also known as abdominoplasty, is designed to remove excess skin and fat from the middle and lower abdomen, and can dramatically improve the size and shape of the abdomen. As such, it is one of the most popular cosmetic surgery procedures.
In addition to removing the surplus skin and fat, other procedures may be added as necessary, including plication (the tightening of the abdominal muscles), and liposuction to remove excess fat elsewhere on the abdomen and waist. Since this is a major surgical procedure, patients who intend to lose weight in the future, or, in the case of women, intend to have children subsequently, should postpone an abdominoplasty.
There are technical variations on the principle of abdominoplasty:
- Standard Abdominoplasty
The surgeon removes the skin and fat from between the pubic bone and umbilicus, and tightens the abdominal muscles. In drawing the skin at the level of the umbilicus down to the pubic level, the umbilicus itself must be brought through the new skin lying over it. This form of abdominoplasty results in a long, curved scar at the pubic level, and also a circular scar around the umbilicus. The procedure is performed under either general anaesthetic, and usually takes about hours, with a one night stay at the Hospital.
- Modified Abdominoplasty – Mini-Tuck
The excess skin above the pubic bone is removed, and the abdominal muscles are be tightened under the umbilicus, with the umbilicus remaining undisturbed. The procedure is performed under either general anaesthetic, and usually takes about 90 minutes, with a one night stay at the Hospital.

Excess fat in certain areas of the body is often extremely difficult to lose, even with determined dieting, and exercise. Men and women with small localised fat deposits (such as outer thigh saddle bags), and those with significant upper or lower body disproportion often have little or no substantive weight problems, but nonetheless find themselves unable to lose this excess fat. Such fat deposits often run in families, and are resistant to diet and exercise. For such patients, surgery is the only option to remedy this condition.
Liposuction, in all its various guises, offers a straightforward, surgical method of removing this excess fat, and is one of the most popular cosmetic surgery procedures. For most men and women, fat removal results in a dramatic improvement in overall body proportion, and enhanced self-confidence.
- Suction Assisted Lipoplasty
This is the term that applied to a group of operations that are intended, in broad terms, to suck out the excess fat content of localised areas on a person’s body. Their purpose is, in almost all situations, to improve the contour or shape of the body. In this sense, the term can be considered to encompass all the following operations:
- Conventional Liposuction
This was the original form of fat removal, and is the technique used by the majority of cosmetic surgeons in the United Kingdom. It usually involves the use of large cannulae, and conventional suction machines. The aim is generally to remove only the deeper layer of fat. As a result, it is often associated with residual fat deposits.
- Liposculpture
This is a more modern refinement of conventional liposuction. It uses far smaller cannulae, and involves the surgeon suctioning fat from much nearer the surface of the skin. As a result, it leaves far less fat behind, and, in experienced hands, produces much smoother, and more aesthetically pleasing results. Furthermore, by producing such thin skin flaps, skin tightening is much more effective and predictable. The suction may be applied either by machine or in a much gentler fashion by a simple hand held syringe (known as superficial syringe liposculpture). By using such small instruments, the surgeon is able to sculpt the body in a gentle, controlled way.
In conventional liposuction, a vacuum machine is used to help the surgeon remove the fat. In liposculpture, a hand held syringe is used. The surgeon uses whichever technique or combination of techniques that he deems most appropriate for each patient.
The procedure may be performed under either local anaesthetic with sedation, or general anaesthetic, and usually takes between 30 minutes to four hours (depending on the volume of fat to be removed). Many patients go home the same day, whereas larger cases require at overnight stay at the Hospital.
Generally, patients are advised that they may not see significant results within the first few weeks. In larger cases, there will be some size difference and most patients should see an improved shape. Patients wear tight support dressings for several weeks. By around eight weeks, most patients achieve about half of their ultimate visible outcome.

Body contouring is the term used to group together those cosmetic surgery procedures that are designed to improve the shape and appearance of the body (excluding the breasts) rather than the face. It encompasses procedures that do one of the following: remove fat; trim or tighten excess fat; enhance muscle appearance.
Modern state of the art of body contouring may involve management of any combination of the following five features:
- Excess fat in certain areas
- Skin sag in other areas
- Poor muscle tone and definition
- Skin appearance, and cellulite in certain areas
- Excess general weight, and obesity
Each of these elements must be dealt with on its own merits. Whilst many patients may only require liposuction, others may need any combination of these features. Each factor will vary in importance for any given patient, and for any given body area on that patient. This is precisely why patients need to see an expert in this field, rather than someone who has a vested interest in ‘selling’ a particular treatment.
An individual may require different treatments to different sites. For example, a person with localised excess fat around the buttocks and thighs, but with saggy skin on the arms (perhaps after determined weight loss) may be best suited to liposuction for the former, and an arm lift for the latter. In addition, any one area may require a combination of procedures. By way of illustration, a tummy tuck with abdominal fat removal would be appropriate for a person with both excess fat and saggy skin on the abdomen (perhaps after pregnancy). The key is to tailor the treatment plan to the patient’s own needs, wishes, and expectations. At all stages, a realistic assessment of what can, and, perhaps more importantly, cannot be achieved must be made clear to the patient.
As one of the most recognisable features of the face, the nose defines our facial appearance. Many men and woman express dissatisfaction with their noses for a number of reasons, including length, width, the presence of humps on the bridge, and breathing problems.
Nose reshaping, also known as rhinoplasty, can address these problems in many patients, and achieve extremely effective results. The best candidates for nose reshaping are patients with thin, non-oliy skin, those who do not regularly use nasal sprays or other nose medications, non-smokers, and those with realistic expectations about the surgery.
The primary aim of nose reshaping is to reduce the size and shape of the bony and cartilaginous framework of the nose, and to re-drape the nasal skin over the altered shape. Occasionally, the patient may only require surgery to correct a deformity of the tip of the nose in what is known as a rhino-tip procedure. It is generally impossible to fully correct a heavy, bulbous nasal tip, over which the skin may be thick and oily.
There are two basic types of rhinoplasty – closed and open (in addition, the internal septum that separates the two sides of the nose may require straightening, a procedure known as septoplasty):
- Closed Rhinoplasty
Tiny incisions are made in the inside of the nose, leaving no external scars. The majority of nose re-shaping surgeries are of this type. Whilst popular with cosmetic surgeons and patients, it has the disadvantage that the surgeon does not have as good a view of the operative field as with Open Rhinoplasty, and which limits the ability to remodel the bone, and cartilage.
- Open Rhinoplasty
An incision is made on the inside of the nostrils, and across the divide of skin between the nostrils – the columella. The skin over the nose is then lifted, which allows the surgeon a clear, uninterrupted direct view of the framework of the nose. As a result, it is easier to make more extensive alterations to the shape of the nose and to provide a more sculpted nasal tip. However, there is the slight disadvantage of the tiny scar across the columella (which is generally barely visible with time), and the fact that any swelling may take longer to subside than with the closed method.
Augmentation Rhinoplasty
Occasionally, patients present with what they consider to be a flat nose, perhaps as a result of injury, previous surgery, or even an inherited or ethnic trait. In order to improve the appearance of a flattened nose, it is necessary to insert some form of additional framework under the skin of the nose to enhance the line of the nasal bridge.
Several options are available for this additional framework, namely using the patient’s own bone (usually taken from the hip, rib, or surface of the skull), the patient’s own cartilage (usually taken from the ears, or from within the nose itself), or using artificial materials, such as silicone implants.
The procedure may be performed under either local anaesthetic with sedation, but most surgeons prefer general anaesthetic. It and usually takes around one hour to 90 minutes, and can be performed as either a day case, or with an overnight stay.
The operation is surprisingly pain-free, and most patients require only simple analgesia, though patients should expect the nose to feel stuffy for several weeks. There may also be visible bruising around the nose and eyes for a few weeks, and areas of the nose, especially the tip, may feel numb for around a month. Following surgery, ice packs may be used to reduce swelling and bruising. For the first few days after surgery, patients are advised to rest quietly, with only gentle activity. Strenuous activity should be avoided for four weeks. More specific advice is included in the relevant patient literature provided by the Hospital.

As we get older, it is common to notice changes in the appearance of our eyes. The eye lids may start to sag, or become puffy as early as our early thirties, and the skin may begin to show excessive wrinkles with advancing years. These changes occur because the skin of the eyelids becomes weaker, allowing the fat that naturally cushions the eyeball to fall forward, creating ‘bags’. This has the effect of making the eyes look smaller, and the face look older and tired. These same effects can be seen in younger patients, often in their twenties, who develop excessive skin and fat during childhood or adolescence, and in patients with eye changes associated with thyroid disease.
Eye lid reduction and eye bag removal, also known as blepharoplasty, is a common cosmetic surgical procedure to remedy these symptoms. It is often combined with a brow lift, or a facelift to achieve a more youthful appearance. Surgery to the upper eye lids is referred to as upper blepharoplasty, and surgery to the eye bags is referred to as lower blepharoplasty. When combined, the purpose is to tighten hooded eyelids, restore their normal contours, and to eliminate fatty bulges. The procedure does not correct fine crow’s feet wrinkles – can be treated very effectively through non-surgical options (such as Botox®) – nor dark circles under the eyes caused by excessive skin pigmentation.
On the upper lid, an elliptical incision is made across the crease of the eyelid, allowing the removal of excess skin, and, in some cases, a thin strip of the underlying muscle. In addition, a small amount of fat may also be removed.
On the lower lid, there are two popular techniques. Traditionally, an incision is made just below the lower lashes, allowing a small ellipse of skin and muscle to be removed, along with the small pads of fat. Where there is a fatty bulge, with no excessive skin, a transconjunctival or incisionless blepharoplasty may be possible. An incision is made, hidden just inside the lower eyelid, healing naturally without the use of stitches.
The procedure may be performed under either local anaesthetic with sedation, or under general anaesthetic, and usually takes around two hours if both upper and lower lids are being operated on. Most cases may be performed as a day cases – not requiring an overnight stay at the Hospital – but an overnight stay may be necessary in some instances.

Ears that protrude prominently can be the cause of much dissatisfaction, and even great unhappiness for men and women who experience this condition. Prominent ear correction/reshaping, also known as otoplasty, is a surgical solution that can produce significant change in one’s appearance, and is considered one of the most successful cosmetic surgery procedures.
The primary objective is to reduce the protrusion of the ears, and to provide a normal configuration when the ear is viewed from the side. This is achieved by modifying the flexibility of the cartilage of the ear, thereby altering the shape and position to a more desirable appearance. The procedure may vary, but usually an incision of 4 to 5 cm is made in the skin in the groove behind the ears, where any remaining surface scar will not be directly visible. Occasionally, incisions are made on the front surface of the ear, although these are placed within the normal contours.

Following surgery, the ears are carefully padded and a head bandage is applied. This is necessary to maintain the newly achieved position of the ears, whilst the initial moulding and healing take place. It also helps to minimise swelling and discomfort. This helps to maintain the position and also to protect the ears from any inadvertent injury which may happen whilst sleeping. Contact sports should be avoided for at least three months after surgery to prevent damage.

For many people, facelift surgery, also known as rhytidectomy, is what comes to mind when they envisage cosmetic surgery. This classic cosmetic surgical treatment can significantly reduce the signs of ageing, and dramatically enhance and rejuvenate a person’s appearance.
The age of men and women interested in facelifts varies, with some relatively young patients wishing to reverse the negative effects of their lifestyles on their facial lines, wrinkles, and sagging, while many older patients desire a facelift to make them ‘look good for their age’. Whereas some symptoms of aging and lifestyle can be treated very effectively through non-surgical options (such as Botox®, and dermal fillers), in many instances a fuller and more permanent remedy can best be achieved through a facelift.
The term facelift covers a number of surgical procedures that do one or more of the following: remove excess skin; remove excess fat; tighten loose skin; tighten sagging muscles. There are several different types of facelift surgery, and certain procedures are more suited to certain patients. Also, different surgeons may specialise in one or another according to their surgical preferences. In addition to the Scarless Suture Method, the different types of facelift include:
- Mini-Lift
This is the most basic of facelifts, and involves little more than a skin tuck. The surgeon lifts the skin from the underlying muscle, bone, and fat, then the skin is stretched taut, and any surplus skin excised, and the wound sutured. Drains may be used.
The mini-lift has little, if any effect on the grooves running between nose and mouth (known as the naso-labial grooves). Depending on the amount of skin removed, the results may be more or less marked, and, consequently, last for varying periods of time.
- SMAS Lift
The Superficial Musculo-Aponeurotic System, or SMAS, procedure involves the surgeon lifting the deeper layer of muscle in addition to the overlying skin. This allows the structures in the lower face and neck to be sutured to bone higher on the face, hence tightening the sling running around the lower face and chin/neck. Any excess fat may also be removed.
- Extended SMAS Lift
In this extension to the conventional SMAS procedure, the surgeon lifts the whole of the skin and SMAS layer all the way to the nose, totally elevating it from the cheek structures beneath. Consequently, the appearance of the mid-face is improved considerably, as are the naso-labial grooves.
- Composite Facelift
This procedure is generally taken to involve an extended SMAS lift, to which a browlift is added, and possibly also lower eyelid surgery.
- Mask Lift
This modern technique is a much more extensive procedure, and is sometimes referred to as Subperiosteal Facelift. It involves a hairband-type incision, which is taken right down through the covering of the bone. This allows all of the deeper layers of the face to be lifted. Incisions within the mouth to release all of the structures of the cheek.
- Endoscopic Lift
This is a refinement of the Mask Lift, allowing for the lifting of the deeper structures of the face to be performed through several small incisions, and through key-hole surgical instruments.
A basic Mini-Lift may take around two hours. Larger face-lifts, especially if combined with other procedures, may take around five or more hours. It is likely the face will be bandaged following surgery. Depending on surgeon preference, and the type of lift, these bandages may remain in place for up to five days. For the first 24 hours or so after surgery, patients are advised to sleep upright. Sutures are usually removed after one to two weeks. Rest at home for the first week following surgery is essential, and strenuous activity should be avoided for about one month, though it is important to take gentle exercise.

In addition to the several conventional facelift procedures, the Scarless Suture Method (SSM) facelift, also known as the Serdev Scarless Suture Method, offers patients contemplating facelift surgery a further level of choice.
The goal of the Scarless Suture Method is to restore the natural proportions, volumes, and angles of the face by raising the face tissues. As a significantly less invasive procedure, where the sutures actually dissolve and are reabsorbed in the skin, the aim is to achieve better aesthetic results, with less scarring.
Rather than the incisions of other facelift procedures, the Scarless Suture Method involves needle skin perforations between predetermined points in the face. These catch movable but stable tissue, and attach it to non-movable tissue using special polycon stitches. These stitches are elastic, absorbable, antimicrobial, and braided, and lift the target area of the face (namely example, the brow, temporal region, or mid/lower face) whilst preserving the area’s natural mimics and movement. The results are intended to rejuvenate the appearance of the face, while keeping its ‘natural look’.
The Scarless Suture Method can be used in the following facelift treatments (all of which are usually performed as day cases – not requiring an overnight stay at the Hospital – under local anaesthetic):
- Brow Lift
A brow lift using the Scarless Suture Method can improve the appearance of eyelids using less invasive means. Results can be immediate, with minimal or no swelling or bruising.
- Temporal Lift, and Mid-Face Lift
This procedure remedies the sagging of the Submuscular Aponeurotic System (SMAS), a condition that frequently results in a sad and tired expression. In older patients, this is frequently seen as jowls in the mandibular area, or a loss of volume in the mid-portion of the cheeks or in a visible naso-labial fold.
In the temporal lift, the effects of the procedure are seen as a tightening and upward and lateral pull of the temporal region.
In the mid-face lift, lifting loose SMAS in this area give a more prominent cheek-bone, with volumes moved to a higher position, and an elongation of the ‘beauty triangle’.
- Lower-Face Life
Like the Temporal, and Mid-Face Lifts, a Lower-Face Lift can remedy SMAS sagging, restoring aesthetic angles and proportions, resulting in a more youthful appearance in the older patients, and an enhancement in the contours and appearance of younger patients. This procedure can be performed in conjunction with other procedures in the face.

Inverted nipples, where a patient’s nipples are not fully emerged from the body (i.e. pointing inwards), are relatively common, affecting some 10% of women. It can be treated extremely effectively with a straightforward surgical procedure, usually conducted under local anaesthetic, lasting approximately 30 minutes. As the procedure is relatively swift, and not intensive, patients are admitted as outpatients, and return home on the day of the procedure, without the need to stay at the Hospital overnight. In most instances, one procedure will be sufficient, however, certain patients experience a reversion in their nipples in the weeks following surgery, which may require a second procedure.
Inverted nipples result from a woman’s milk ducts in the breast being too short as they enter the nipple. Correcting the condition involves the surgeon drawing out the inverted nipple through making a small incision to the lower outer rim of the nipple, and then either carefully stretching the milk ducts leading to the nipple, or dividing them. In cases where the milk ducts are stretched, subsequent breastfeeding is normally possible. Where the milk ducts need to be cut, breastfeeding can be affected. Based on each patient’s individual circumstances, your surgeon can provide specific advice pertinent to you about breastfeeding.

Breast enlargement, also known as breast augmentation or mammoplasty, is by far the most popular cosmetic surgery procedure requested by women. Though a relatively straightforward operation, it can have a truly transformative effect on a woman’s appearance, and self-confidence.
A number of factors can cause women to be concerned about the appearance of their breasts, including unhappiness with the size or shape of their breasts, the loss of breast tissue and size (for example, following pregnancy, increasing age, or weight loss), and congenital underdevelopment or absence of one or both breasts. For many women, these factors can be remedied very effectively through breast augmentation.
The surgery involves placing an artificial implant into the breast in order to increase breast size, and, where necessary, to produce a degree of lift. Most patients are admitted on the morning of the day of their surgery, and discharged the next day after spending the night at the Hospital, though some patients are treated as day cases.
Breast implants have a finite period of recommended use. Though some implants can last for decades without any issues arising, the replacement of implants around 10 years after surgery is advised. In addition to the durability of the implants, changes in a woman’s body in the period following surgery can make an additional procedure beneficial, and, in some instances, necessary.

Our History
With a history going back over a century, patient care is at the heart of the heritage of the Alexandra Private Hospital. Built in 1908, the site of the Alexandra was originally part of the Chesterfield Royal Hospital.
In 1984, the Royal Hospital was re-located to the outskirts of the town, and the original hospital was purchased by consultant orthopaedic surgeon, Mr. S. Sherif (Fellow of the Royal College of Surgeons Ed., Glas.) who had previously practised as a surgeon at the Royal Hospital. Mr. Sherif began the wholesale renovation of what was to become the Alexandra Private Hospital, converting it into the modern private hospital that it is today.
Whilst its beautifully maintained exterior retains all the splendour of its past, the Hospital’s interior boasts the most up-to-date facilities, offering a level of private medical care unrivalled in the region.

When sagging of a woman’s breasts becomes prominent, a breast uplift, also known as mastopexy, is often an appropriate remedy. Sagging of the breasts is very common, and can be caused by a number of factors, including age, the effects of gravity, significant weight loss, and pregnancy and breastfeeding. The negative impact that this change in appearance can have on a woman’s self-esteem leads many women to consider surgery to reverse the effects.
Through a breast uplift, the size and shape of breasts can be improved, giving a more natural, youthful appearance. The surgery involves the removal of surplus skin caused by drooping. The breasts are then re-shaped, and the nipple/areola complex is re-positioned to a higher level so that the nipple lies at the point of the newly tightened breasts. Where necessary, the surgeon may reduce the size of the areola to achieve an enhanced, more natural appearance. The procedure is performed under general anaesthetic, usually takes between 90 minutes to two hours, and normally involves a one night stay at the Hospital.
Many women undergoing breast uplift surgery also decide to increase the size of their breasts by insertion of breast implants (see Breast Enlargement). This can be performed at the same time as the breast uplift, rather than requiring the patient to be anaesthetised and undergo surgery twice.

Breast reduction surgery, known as reduction mammoplastly, is often an appropriate solution for women who experience the difficulties caused by overly large breasts.
The size of a woman’s breasts is determined by several factors, including genetics, body weight, and hormones. As such, overly large breasts may become an issue for women at different stages of life. At whatever age that it affects a woman, there are certain consequences that are common to all situations, such as discomfort, neck and back pain, rashes and soreness under the breasts, and difficulty in finding appropriately sized bras and other items of clothing. In certain circumstances, women can also find overly large breasts to be an impediment to how they live their lives (such as those who wish to pursue sports), and can affect their self-confidence extremely negatively.
Breast reduction surgery involves the removal of excess skin, and fat from the breasts. The breasts are then re-shaped to suit their reduced size, and the nipple/areola complex is re-positioned to form smaller, more naturally shaped breasts. Asymmetries between the size of the breasts can also be corrected during the procedure. Where required, liposuction may be performed on fatty areas under the armpits. The procedure is performed under general anaesthetic, usually takes between two to four hours, and normally involves a one night stay at the Hospital.
In the most common procedure, scars will result around the areola, then vertically down, and sometimes also horizontally out along the breast creases. Following surgery, a firm support bra needs to be worn for between four to eight weeks. Depending on a patient’s specific circumstances, a support bra may need to be worn indefinitely.
Generally, it is advised that most women will not be able to breast feed following breast reduction surgery, however, where the connections between the milk ducts and the nipple can be preserved, breast feeding may be possible.
